North Western Melbourne Primary Health Network (NWMPHN) has developed a comprehensive toolkit to help aged care homes better support residents during the after-hours periods.
It allows staff to assess current preparedness, keep resident medical records up to date, prepare action plans for each resident, communicate with relevant family members, and compile a directory of local health, welfare and emergency services.
The toolkit is the result of consultation with the aged care sector, and collaboration with South Eastern Melbourne Primary Health Network, Ambulance Victoria and Victorian Virtual Emergency Department.
The toolkit can be download from this page. We recommend you download and store the resources where you keep your specialty documents. It comprises:
The directory below comprises names, times and contact details for dozens of organisations that provide urgent health care outside standard operating hours. Click the dropdown menus to view contact details. A downloadable version is available here (.doc).
We encourage you to identify relevant services in your region – including local ones such as locum and after-hours pharmacy services – and record them using our downloadable template.
