Primary health networks were funded by the Australian Government to develop dementia resources, as a response to the Royal Commission into Aged Care Quality and Safety.
This directory and accompanying information sheet have been developed for people who live in the northern, western and eastern suburbs of Melbourne.
The information sheet is designed to be given to people when they are diagnosed, so they may refer to it later. It includes information about all the key organisations that can help, including Dementia Australia, Carer Gateway,
My Aged Care, and the NDIS.
