The dementia consumer resources and directory have been developed as part of the Dementia Support Pathways and Implementation Project. This activity is supported by funding from the Australian Government, through the PHN program.
The aim of this initiative is to support people living with dementia to have better access to dementia care and live well in the community, for as long as possible.
