Our system of care promotes a person-centred approach to mental health, suicide and alcohol and other drug treatment and care. It supports services that are responsive to community needs and expectations.

Services commissioned under our system of care deliver targeted, appropriate services to a wide range of community and cultural groups at numerous locations across our region.

Services in this directory are presented according to age group, target population, location, treatment type and level of vulnerability to assist people to find the best service for their individual needs.

Psychological Treatment for People in Residential Aged Care Homes

Low to medium intensity psychological treatment service for aged care residents.

Better Place Australia delivers services to residents of aged care homes located in the north part of the NWMPHN region, encompassing these local government areas: Darebin, Hume, Merri-bek, Yarra, Melbourne, and Macedon Ranges (Gisborne part*).

APMHA Healthcare delivers services to residents of aged care homes located in the west part of the NWMPHN region, encompassing these local government areas: Brimbank, Hobsons Bay, Melton, Maribyrnong, Moonee Valley, Wyndham, and Moorabool (Bacchus Marsh part*).

*Some parts of the Macedon Ranges and Moorabool local government areas are outside the NWMPHN region and so are not included in this program. View a map of our region here.

Target populations: Residents living in residential aged care homes in the NWMPHN region.

Stepped Care for Older Adults | Merri Health

Professional mental health support for older adults, with mild to moderate mental illness or at risk of developing a mental illness, due to social isolation, loneliness, or life stressors.

Target populations: People aged 65 years and over (50 years and over for people who identify as Aboriginal and/or Torres Strait Islander) with mild to moderate mental illness or are at risk of developing a mental illness.

Locations: Bacchus Marsh, Brimbank, Darebin, Gisborne, Hobsons Bay, Hume, Maribyrnong, Melbourne, Melton, Moonee Valley, Moreland, Wyndham, Yarra

Head to Health Victoria | Wyndham Vale, Broadmeadows and Brunswick East

Head to Health provides advice and a welcoming place for everyone who needs mental health and wellbeing support. It is available to all Victorians of all ages.

The service provides care navigation, access to a multidisciplinary mental health team at Head to Health hubs, and a team-care approach, to complement and supplement GP care.

Target populations: Victorians of all ages

CAREinMIND Targeted Psychological Support Services

Focussed psychological therapies delivered by qualified mental health practitioners, for people with mild to moderate diagnosed mental health issues.

People generally receive up to 12 face to face appointments/ sessions per year in this service.

Services providers include: CAREinMIND, Drummond Street Services, Drummond Street queerspace, Merri Health, MIND Australia – Fitzroy, Thorne Harbour Health, Macedon Ranges Health and individual practice providers.

Target populations:

People with a diagnosed (mild to moderate) mental illness (includes clients under the age of 18 with a provisional diagnosis).

Mental Health Services for Aboriginal and Torres Strait Islander Adults | Victorian Aboriginal Health Service

Focussed psychological therapies delivered by qualified mental health practitioners, for Aboriginal and Torres Strait Islander people with mild to moderate diagnosed mental health issues.

People generally receive up to 12 face to face appointments/sessions per year in this service.

Target populations:

Aboriginal & Torres Strait Islander people with a diagnosed (mild to moderate) mental illness.

Locations: Preston

headspace Collingwood | headspace

headspace provides young people with a range of integrated mental health, alcohol and other drugs, physical and sexual health and vocational supports.

Young people generally receive a flexible number of sessions based upon their needs.

Target populations:

Young people and their family to access help for problems affecting their wellbeing.

Locations: Abbotsford
