The North Western Melbourne Primary Health Network region is one of the most complex and diverse in Victoria. Groups in the region with disproportionately high rates of chronic disease and poorer overall health are designated ‘priority populations’.
They include culturally and linguistically diverse groups, LGBTIQ people, those experiencing homelessness, and Aboriginal and Torres Strait Islander peoples, as well as other priority groups. Each priority population has additional and complex needs that are not necessarily being met by the current service system.
