2021 Community Health Needs Provider Survey

Have your say about community health needs to inform how we plan and fund health services.

Our 2021 Community Health Needs Provider Survey has now closed.

Thank you for your input. Your responses, and those of other people in our region, will be used to help us understand what health services are needed.

About the survey

One of our roles as a Primary Health Network (PHN) is to plan and fund health services that are needed in our region.

Every three years we complete a Health Needs Assessment. This involves looking at health data and talking with our local communities, health care organisations and other stakeholders to better understand the needs of people in our region.

How you can help

We are interested in hearing from people who work in the health sector or any other sectors regularly working with at risk and/or disadvantaged populations in the North Western Melbourne Primary Health Network (NWMPHN) catchment. This will assist us to develop a holistic picture of our communities’ health needs.

The survey will take approximately 15 minutes to complete and all information provided will remain anonymous.

NWMPHN along with its partners will use the information to better identify health priorities for the region.

After you complete the survey, we encourage you to share it with your colleagues in our region. Use this link to quickly send your colleagues an email about the survey.

Your answers help us to plan better health care in our region

The responses from this survey will be analysed along with:

  • responses from a community survey
  • health data (for example, data from the Australian Bureau of Statistics, hospitals and general practices)
  • consultations with community groups, service providers and peak organisations.

Our Reference Group (an internal working group) and our Community and Clinical Councils will review the data and discuss priority areas and how we might address these.

The report of all these findings becomes our Health Needs Assessment. We will use this to work with our communities and service providers to design and improve health services and activities.

North Western Melbourne Primary Health Network is a not-for-profit organisation funded by the Australia Government.

Our role is to help improve the health of people living in central, northern and western Melbourne. We do this by planning and funding health services that communities need.
