Heart and vascular diseases are some of the most common chronic conditions in the North Western Melbourne Primary Health Network (NWMPHN) region.
This upcoming Heart Week (6-12 May), everyone is encouraged to ‘never miss a beat’.
Health professionals can use the Heart Foundation’s Aus CVD Risk calculator to deliver more heart health checks for at-risk patients, and:
- Register for a clinical webinar on Thursday, 9 May 2024, 7-8:30pm, titled ‘Metabolic matrix: introducing the cardiovascular-kidney-metabolic (CKM) syndrome’.
- Download or order the Heart Week resource pack for a variety of resources and promotional tools to assist in engaging patients about their heart health.
- Enter the Heart Week display competition, using the resource pack and decorations to set up a heart health display in general practice and submit a photo for the chance to win one of 2 $500 vouchers.
- Use the updated CVD risk assessment and management templates aligned to the 2023 Australian CVD risk guideline.
General practices can also use NWMPHN’s heart-related Quality Improvement activities:
- Increase cardiovascular risk recording in your practice.
- Increase the number of absolute cardiovascular disease (CVD) risk assessments for Aboriginal and Torres Strait Islander patients.
- Improve identification of patients with heart failure in general practice through data management.
Community members with any concerns are encouraged to discuss these with their GP, or learn more about heart health on the Heart Foundation website.
