How to introduce the Healthy End of Life (HELP) app to patients, clients and carers.

The HELP Brief Intervention is a quick and simple way to activate network-centred care for people in palliative and aged care settings. Network-centred care improves end of life outcomes and the HELP app facilitates this in a practical, easy to use way.

This workshop will cover:

- What is network-centred care and how does it improve end of life experiences

- How to activate informal network supports for clients and patients

- Implementing the HELP Brief Intervention in your professional practice

- Barriers and trigger points for using the app.
