Vulnerable peoples homebound COVID-19 vaccination program

North Western Melbourne Primary Health Network (NWMPHN) is accepting referrals from health care agencies and providers, aged care providers, general practices, and health communities within its catchment for in-reach home visits to provide COVID-19 vaccinations to people who cannot leave the home, including residents in aged care homes.

If you have a patient or know of someone who cannot leave their home to receive a COVID-19 vaccination, please send a request for vaccination using the link provided below.

The Vulnerable Peoples Homebound Vaccination Program (VPHVP) is an Australian Government initiative to provide vaccinations to vulnerable populations. The homebound may include but are not limited to the frail aged and elderly, aged care residents, and people living with a disability or a mental health condition which prevents them from leaving their home.

Once the referral has been received, a vaccination provider will contact the homebound person (or nominated contact person) to schedule a time for the in-reach home visit.

All requests must come through the referral link provided to be scheduled for a vaccination.
