  • Information of new Voluntary Assisted Dying Legislation Act 2017

  • Discuss Advance Care Planning with patients and their carers/families appropriately and clearly
  • Source Advance Care Directives documentation
  • Develop an approach to referring patients to Palliative Care Services
  • Respond to requests from patients for information about or access to Voluntary Assisted Dying
