Your Community Health in Darebin has launched a new MBS Podiatry Clinic, which is accepting referrals for clients with chronic conditions like diabetes, peripheral arterial disease, neuropathy and more.
There is no waitlist and clients are offered an appointment within a fortnight of referrals being received. Health Care Card and pension card holders are bulk billed, and other clients are eligible for a Medicare rebate with an out-of-pocket cost of $10.
To refer your client to Your Community Health’s Podiatry Clinic, please complete:
- a Team Care Arrangement;
- a ‘Referral Form for Individual Allied Health Services under Medicare for patients with a chronic medical condition and complex care needs’, noting the allocated number of podiatry appointments.
