In order to improve the quality of outpatient referrals and minimise the declining of referrals, the Royal Melbourne Hospital (RMH) has developed a new standard GP referral smart form to assist referring doctors.
The new referral form contains a list of all specialties that are available to refer patients and includes the Head of Unit name to allow more choice of clinics to refer your patients to. For example: MBS and non-MBS clinics.
The PDF form can be completed online, include any attachments, print and fax to Outpatient Specialist Clinics or the Direct Access Unit for sub-acute clinics. All fax numbers are on the referral form.
