Our second round of the QI Grants program is winding up at the end of March. A total amount of $57,000 has been funded to 13 general practices for projects covering:
- chronic disease management
- family violence
- LGBTIQ
- health literacy
- youth mental health
- Hepatitis C
- family planning
- advance care planning
On 16 March, participants gathered together at NWMPHN in Parkville for a final learning workshop where they presented their projects, ideas and learnings. So far, the PHN has seen an increase in data quality, improved patient outcomes and better community access to health services.
