New HealthPathways are now available to help GPs assess, manage and refer patients needing palliative care.
Palliative care patients are mostly managed in primary care and may require different levels of involvement from specialist palliative care services depending on their needs and the capacity and capability of their primary care provider. Patients may move between levels during the course of their illness and may receive palliative care in any setting.
