In 2023-24, North Western Melbourne Primary Health Network (NWMPHN) commissioned and managed more than 350 contracts, and collaborated on hundreds more projects across our region, state and country.

Our work supported health care providers located across our 2,300 square-kilometre catchment and beyond.
These included more than 550 general practices, the vital anchors of our system, which also includes care providers working in allied health, community health centres, public and private hospitals, mental health services, urgent care clinics, pharmacies and aged care homes.
All of these play major roles in helping the 1.9 million people who live in Melbourne’s CBD, western and northern suburbs access the health care they need at the right place and time.
One of the most important aspects of NWMPHN’s work and vision is the pursuit of health equity.
At every age, in every community, everyone has a right to expect equitable access to health care. To help put this principle into practice, we research and map health care needs across our catchment. We collate data and listen to the advice of health care workers and consumers.
The annual report reveals how this approach builds new pathways for child health, boosts collaborations with First Nations health organisations, and helps older people retain independence.
