Let’s talk about the Health Care Home

North Western Melbourne Primary Health Network (NWMPHN) supports the Health Care Home model which places patients with chronic and complex health conditions at the centre of care that is coordinated through their GP.

While not a trial site for a Health Care Home, NWMPHN CEO Adjunct Associate Professor Christopher Carter said NWMPHN will observe the roll-out and implementation of the Health Care Home trials to see what works and what doesn’t.

“We are already working towards implementing the principles that underpin the health care home model in our region, but what is essential for us is to also stimulate discussion on the subject,” Adjunct A/Prof Carter said.

“The recent release of our discussion paper The Heath Care Home: What it means for Australian Primary Health Care aims to precisely do that – engage the health sector and exchange thoughts on new models of care that benefit the community.”

“Delivering quality improvement and care is the business of all health professionals,” Adjunct A/Prof Carter said.

“As a Primary Health Network it’s our imperative role to actively encourage discussion in this space with the goal to commission services that provide the right care, at the right time and at the right place.”
