GPs: help strengthen palliative care in the North Western Melbourne PHN region - North Western Melbourne Primary Health Network

Do you manage patients at end of life? Do you work with community palliative care providers?

North Western Melbourne Primary Health Network (NWMPHN) is working with the North and West Metropolitan Region Palliative Care Consortium to better understand how these interactions and relationships can be improved. We want to hear about how community palliative care services work with GPs when providing palliative and end of life care to patients in the community.

This is an opportunity to strengthen collaboration, learn about what is working well and identify opportunities for improvement.

We encourage GPs providing palliative care in the NWMPHN region to complete the following 10-minute survey. Results will be communicated in our General Practice Update.
