General practices invited to join new chronic disease clinical trial

The University of Melbourne’s Department of General Practice and Primary Care, on behalf of the Western Health Nephrology & Endocrinology units, is delighted to invite general practices to participate in a new clinical trial.

The trial is focused on preventing or delaying progression of chronic disease in our community, with chronic kidney disease and diabetic care as key priorities.

It will explore a model of care that combines electronic clinical decision support with enhanced support from the Western Health team, and is available to practices in the Western Health catchment.

What will practices be asked to do?

The project will ask GPs to use an innovative clinical decision support technology called Future Health Today (FHT) in their daily practice. FHT will be provided to the practice free of charge for the duration of the project.

In addition, Western Health clinicians will provide clinical education and support, access to a chronic kidney disease nurse practitioner and increased diabetes educator support.

Project outcomes will be evaluated using data from your practice, securely extracted using the GRHANITE™ tool.

We hope that participation in this project improves diagnosis and management of chronic kidney disease, diabetes, and cardiovascular disease across primary and secondary care, improves collaboration between general practices in the western suburbs of Melbourne and Western Health and, most importantly, improve outcomes for patients.

This project is funded by Safer Care Victoria via the 100,000 lives initiative and includes a $1,000 payment for participating practices.
