NWMPHN is seeking a general practitioner, practice nurse or allied health professional from the Darebin area to join the Living Well Project working group.
The group will focus on patients with chronic diseases that are at increased risk of frequent hospitalisations due to the the complexity of their needs and high service utilisation.
The working group will also use their collective knowledge to drive the Living Well Project for pre-complex patients.
This will include providing leadership and guidance in the development and implementation of a model of care for pre-complex patients to decrease their risk and avoid unnecessary hospitalisation.
