Changes to submitting forms to the Australian Immunisation Register through Health Professional Online Services

From January 2022, you will no longer be able to mail or fax your forms to the Australian Immunisation Register (AIR). Completed forms can be submitted using your Provider Digital Access (PRODA) account to access the Form upload function in Health Professional Online Services (HPOS).

You can submit the following forms through HPOS:

  • AIR – immunisation medical exemption form (IM011)
  • AIR – immunisation history form (IM013)
  • AIR – immunisation encounter form (IM018)

The quickest and easiest way to record immunisation information on the AIR is to go online to the AIR site in HPOS or by using Practice Management Software. Immunisation encounters, medical exemptions and immunisations given by another provider including those performed overseas can all be recorded directly on to the AIR site and the individual’s record will be updated immediately.
