Join a research study aimed at enhancing medication-related communication during transitions of care for aged care home residents.
A team of Monash University researchers are inviting aged care homes to participate in a study dedicated to improving medication-related communication during transitions of care for residents.
Participation will involve assisting the research team in identifying eligible participants (residents, family caregivers, and healthcare providers) for interviews to delve into their experiences with medication communication during residents’ transitions of care. This information will inform the development of interventions tailored to the needs of all parties involved.
Potential benefits: By developing strategies to enhance medication communication, the project aims to mitigate medication-related harm and hospitalisations among residents, aligning with national health care priorities and key indicators for aged care homes.
The closing date for expressions of interest is 31 December 2024.
