Inviting GPs, nurses and care coordinators working in residential aged care to an event focused on navigating difficult conversations in end-of-life care. This event is designed to bring together healthcare professionals working in a residential aged care home. This event aligns with RACH Quality Standards which emphasises the importance of delivering person-centred and coordinated care at the end of life, ensuring that the needs and preferences of older people are met with dignity and respect.
Communicating with patients and families at end-of-life is challenging and requires a distinct set of skills. Regardless of experience, many clinicians do not feel they have these or are uncomfortable discussing prognosis and end-of-life issues. This session will help you approach these conversations with greater confidence, skill, and care, ensuring that patient needs, values and preferences are at the heart of decision-making.
This in-person workshop provides a space for GPs visiting RACH and RACH nurses to work together and build communication skills for end-of-life conversations with residents and their families.
We encourage GPs and RACH nurses to connect and register to strengthen their approach to end-of-life care through this capability-building event.
