Please join us as we discuss frailty and how Palliative Aged Care Outcomes Program (PACOP) informs the palliative and end of life care of your residents.
The PACOP tools assist staff to plan care that is responsive to the needs of the resident and their family/carer. The Rockwood Clinical Frailty Scale (RCFS) is one of five assessment tools used in the PACOP Profile Collection. RCFS supports staff in anticipating changes in care needs, guiding staff in the provision of timely and appropriate care.
