For general practitioners, residential aged care facility managers and care coordinators working across Melbourne

Join us as we follow Betty's story, identifying her palliative care needs, and understanding how and where to access resources. It will look at the palliative care system and cover how to communicate and collaborate with service providers to coordinate optimal end-of-life care.

During the day you will be introduced to the palliative care system and learn how to communicate and collaborate with service providers to co-ordinate optimal palliative and end of life care.

There will be three panel sessions with Q&A opportunities for the audience to participate in and increase knowledge and confidence in managing palliative care needs.

Moderated by Jacinta Parsons, broadcaster, writer and public speaker who currently hosts the Afternoon program on ABC Radio Melbourne.

North Western Melbourne PHN is offering grants to assist with the cost of attending the Communication, Collaboration and Coordination: The three C's of Palliative Care in Aged Care summit. Anyone working in a general practice or residential aged care facility within the NWMPHN catchment is eligible for reimbursement of their registration fee. Proof of summit attendance must be submitted with an invoice for the $75 reimbursement. Submissions must be sent to education@nwmphn.org.au by 5pm on Friday 2 June 2023.
