One day online Chronic Disease Care Planning workshop. The online Chronic Disease Care Planning workshop involves lecture and interactive activities around the application of chronic disease care planning in primary health care.

The Chronic Disease Care Planning workshop aims to provide participants with the skills and knowledge required to manage the patient with chronic disease by including care planning, identifying risk factors, identifying and supporting behaviour change, engaging the patient, applying health interventions and developing effective care plans.

Pre-requisite:

Participants must complete the Foundations of Chronic Disease prior to entry to this program.

Learning outcomes:

Upon successful completion of the program participants should have the ability to:

  • Determine the persons chronic disease management needs, identify risk factors, understand engagement
  • Support the person in behaviour change, recognise the need for referrals, recognise the needs for follow-up and review
  • Apply effective care planning techniques to support the management process.
