Overview
North Western Melbourne Primary Health Network commissions Youth Projects to deliver location-based after hours primary health services for people experiencing or at risk of homelessness.
Services are provided by assertive outreach and in-reach, and at the Youth Project’s drop-in clinic in the City of Melbourne. Services are available from 4pm – 11pm, Wednesday to Sunday, and 11am – 5pm on Saturday (drop-in clinic).
Youth Projects has a strong multidisciplinary team with skills and expertise in addressing a broad range of health issues. This skillset is the ideal match for the diverse and complex physical, mental and social health needs of the people who utilise this service.
Outcomes
The service:
- provides physical and mental health care to people who are or at risk of experiencinghomelessness
- improves health and wellbeing outcomes for people experiencing homelessness
- supports referrals to culturally safe services that are responsive to complex and intersectional needs, including referrals to GPs and other health services delivered during regular operating hours at the Living Room
- engages clients in a safe and non-judgemental space where they can participate in social and health promotion activities.
