Use the QI template below to boost general practice referrals to optometrists for patients living with diabetes.

Goal

Increase the proportion of patients living with diabetes who have an updated GP Management Plan (GPMP), including eye tests.

Measures

Compare Pen CAT4 data for eligible patients at the start and end of the 12-month period, noting the increase in the subset with an updated GPMP.

Improvement ideas

Review GPMP templates to ensure that there is a section for eye health. If eye health screening is missing, update your templates and make them available in your clinical software.
Add eye health screening questions to care plans and heart health assessments.
Send reminders to patients due for an eye health check with their nominated optometrist.
