Enrich Program | Orygen
Enrich is a partnership model program operating out of Sunbury Cobaw Community Health. The program employs senior mental health practitioners who are supported with specialist skills from Orygen, including psychiatry.
Target populations: People with a diagnosed mental illness in the Sunbury and Macedon regions.

Head to Health Victoria | Wyndham Vale, Broadmeadows and Brunswick East
Head to Health provides advice and a welcoming place for everyone who needs mental health and wellbeing support. It is available to all Victorians of all ages.
The service provides care navigation, access to a multidisciplinary mental health team at Head to Health hubs, and a team-care approach, to complement and supplement GP care.
Target populations: Victorians of all ages

CAREinMIND Intensive Support Service
Key aspects of the service are clinical nursing services, medication support, care co-ordination and liaison (clinical and non-clinical services), family support and liaison, improving access to psychiatrist and psychological care.
Service providers include: Australian Primary Mental Health Alliance, cohealth, Empathy Healthcare, Evans Street Medical Clinic, headspace Sunshine, Sage Health, Westgate Health Co-op, Your Community Health and individual practice providers.
Target populations:
People with severe mental illness and other complex care needs suitable to be supported in the primary care setting.

Aboriginal Youth Enhanced Mental Health Program | Victorian Aboriginal Health Service
The Aboriginal Youth Enhanced Mental Health Program provides treatment and support to young Aboriginal people experiencing, or at risk of, severe mental health issues.
It provides culturally safe mental health support that improves wellbeing for Aboriginal youth through child psychiatry practices, psychological therapies, and tailored case management.
Target populations:
12-25 year old Aboriginal young people who reside in the NWMPHN catchment.
Locations: Darebin, Hume, Maribyrnong, Melbourne, Merri-bek, Moonee Valley, Yarra

Trauma informed, early intervention and support including triage for referral to tertiary emergency settings, rapid response for brief interventions and short and medium term assertive outreach and support.
The number of sessions available is flexible based on an individual’s complexity of need.
Target populations:
12-25 year olds who are homeless or at risk of homelessness.
