Our system of care promotes a person-centred approach to mental health, suicide and alcohol and other drug treatment and care. It supports services that are responsive to community needs and expectations.

Services commissioned under our system of care deliver targeted, appropriate services to a wide range of community and cultural groups at numerous locations across our region.

Services in this directory are presented according to age group, target population, location, treatment type and level of vulnerability to assist people to find the best service for their individual needs.

CAREinMIND Targeted Psychological Support Services

Focussed psychological therapies delivered by qualified mental health practitioners, for people with mild to moderate diagnosed mental health issues.

People generally receive up to 12 face to face appointments/ sessions per year in this service.

Services providers include: CAREinMIND, Drummond Street Services, Drummond Street queerspace, Merri Health, MIND Australia – Fitzroy, Thorne Harbour Health, Macedon Ranges Health and individual practice providers.

Target populations:

People with a diagnosed (mild to moderate) mental illness (includes clients under the age of 18 with a provisional diagnosis).

Locations: Whole region

CAREinMIND Intensive Support Service

Key aspects of the service are clinical nursing services, medication support, care co-ordination and liaison (clinical and non-clinical services), family support and liaison, improving access to psychiatrist and psychological care.

Service providers include: Australian Primary Mental Health Alliance, cohealth, Empathy Healthcare, Evans Street Medical Clinic, headspace Sunshine, Sage Health, Westgate Health Co-op, Your Community Health and individual practice providers.

Target populations:

People with severe mental illness and other complex care needs suitable to be supported in the primary care setting.

Locations: Whole region

CAREinMIND Suicide Prevention Service | CAREinMIND providers

Intensive, short term psychological services for people who have been assessed as being at increased risk of self-harm or suicide and are being supported in community.

People generally receive 6-8 appointments/sessions in this service.

Target populations:

People who have self harmed, attempted suicide or who have suicidal ideation.

Locations: Whole region

Connexions | Jesuit Social Services

Counselling, casework and advocacy for young people. The program seeks to engage young people by establishing relationships built on trust and understanding.

People generally receive a flexible number of sessions to meet their needs.

Target populations:

Young People (16-28 years) with high and complex needs, particularly individuals struggling with concurrent mental health and substance misuse.

Locations: Collingwood, Sunshine

Reconstructing Life After Dependency (RLAD) Program | VincentCare

AOD counselling and care coordination.

People generally receive a flexible number of sessions to meet their needs.

Target populations:

Men experiencing homelessness or at risk of homelessness. Men with a history of long term homelessness and substance use.

Locations: Fitzroy

Muslim Youth, Adult and Families Program | Odyssey House

AOD services including counselling, youth outreach, case management and care coordination.

People generally receive a flexible number of sessions to meet their needs.

This service is a collaboration between Odyssey House, Salvation Army, SHARC and YSAS.

Target populations:

Muslim community with AOD substance use issues.

Q Health | Merri Health

An integrated LGBTIQ service that responds to the unique and complex AOD substance use issues and needs of LGBTIQ communities.

People generally receive up to 12 sessions per year in this service.

Target populations:

LGBTIQ communities with AOD substance use issues.

Locations: Brunswick, Coburg

Illicit Drugs & Alcohol Treatment Counselling Services | Australian Vietnamese Women’s Association

Drug and alcohol counselling, consultancy and continuing care for the Vietnamese community. Delivered within the context of harm minimisation and a focus on psychosocial treatment.

People generally receive a flexible number of sessions to meet their needs.

Target populations:

Australians of Vietnamese background and their families.

Locations: Braybrook, Richmond

AOD Program in the North & West | Victorian Aboriginal Health Service (VAHS)

Treatment includes intake and assessment, counselling, care coordination and case management and referral for the Aboriginal community.

People generally receive a flexible number of sessions to meet their needs.

Target populations:

Aboriginal people with substance misuse issues.

Locations: North Melbourne, Preston, West Melbourne

Parallel Pathways | Youth Projects

Healthcare and support to improve the physical, mental and social wellbeing of individuals who are homeless or at risk of homelessness and are experiencing comorbid AOD substance misuse and mental health concerns.

People generally receive a flexible number of sessions to meet their needs.

Target populations:

Individuals with coexisting mental health and AOD experiencing primary or secondary homelessness.

Locations: Melbourne

headspace Collingwood | headspace

headspace provides young people with a range of integrated mental health, alcohol and other drugs, physical and sexual health and vocational supports.

Young people generally receive a flexible number of sessions based upon their needs.

Target populations:

Young people and their family to access help for problems affecting their wellbeing.

Locations: Abbotsford

Mental Health Services for Aboriginal and Torres Strait Islander Adults | Victorian Aboriginal Health Service
