
North Western Melbourne Primary Health Network (NWMPHN) is proud to release its Reconciliation Action Plan (RAP) for 2025–2027.
It is NWMPHN’s third ‘innovate’ RAP – an important part of the five-stage transformation framework mapped out by Reconciliation Australia.

It reaffirms our commitment to reconciliation and improving health outcomes for Aboriginal and Torres Strait Islander communities.
“We recognise that reconciliation is an ongoing journey that requires us to remain dedicated to taking meaningful actions that contribute to positive change,” says NWMPHN chief executive officer, Christopher Carter.
“This latest plan – the result of extensive consultation with Aboriginal health organisations and First Nations communities – strengthens our focus on collaboration, co-design, and listening to ensure better, culturally safe outcomes.”
Mr Carter thanked NWMPHN’s dedicated RAP working group, which led the internal reconciliation process.
“The launch of the new plan underpins the fact that reconciliation is everyone’s responsibility,” he added.
“We encourage all our staff, partners, and stakeholders to actively engage with the RAP, and to ensure cultural safety, listening, and health equity inform everything we do.”
Activities in this RAP include refining the NWMPHN engagement framework to provide effective, appropriate, and culturally sensitive principles.
It commits to extending and refining employment plan, creating recruitment opportunities and ensuring that Aboriginal and Torres Strait Islander peoples are well-supported in primary health care workplaces.”
