Would your general practice like to take part in a new mental health care navigation research trial?
In partnership with the University of Melbourne, North Western Melbourne Primary Health Network (NWMPHN) is one of three PHNs in Australia running the Link-me trial. The trial offers eligible patients with complex mental health needs the opportunity to access tailored support through a mental health care navigator based in general practice.
The benefits for the practices involved include:
- GPs will have the option to participate in an Active Learning Module to obtain Category 1 (40) RACGP points
- The practice will be reimbursed a monthly participation grant of $500
During the care navigation sessions, a mental health treatment plan (MHTP) will be developed in collaboration with the GP, and the navigator will support the patient to identify services that are in line with the goals of the MHTP. Care package funds are available through NWMPHN for patients experiencing financial barriers to access the services they need.
