North Western Melbourne Primary Health Network (NWMPHN) is committed to reconciliation and improving health outcomes for Aboriginal and Torres Strait Islander communities.

A key way of achieving this is through the development and implementation of reconciliation action plans (RAPs), aligning with part of the five-stage transformation framework mapped out by Reconciliation Australia.
In March 2025, the organisation published its third consecutive plan, the Reconciliation Action Plan for 2025–2027.
It reaffirms NWMPHN’s commitment to reconciliation and improving health outcomes for Aboriginal and Torres Strait Islander communities. It is a practical commitment to collaboration, co-design, and listening to ensure better, culturally safe outcomes.
Each RAP commits NWMPHN to four key areas:
- Relationships: We partner with local Aboriginal and Torres Strait Islander communities to better understand local health issues. We work together towards ‘closing the gap’ in terms of access to health care and health outcomes.
- Respect: By acknowledging and understanding our history and its effects on the health outcomes of local Aboriginal and Torres Strait Islander communities, we build and develop long-term relationships based on respect.
- Opportunities: We create opportunities to improve health outcomes for Aboriginal and Torres Strait Islander people and develop more inclusive work environments.
- Governance: We have a RAP working group and RAP champions. These are accountable, responsible and ensure we have resources to achieve delivery and evaluation of reconciliation principles.
