Primary care feedback wanted for diabetes remission clinic

The Royal Melbourne Hospital (RMH) is seeking the views of primary care providers on a hospital-based diabetes remission service.

In 2023, the RMH set up the Diabetes Remission Telehealth Rapid Access Clinic to look after adults diagnosed with diabetes within the previous two years.

The aim is to achieve remission through weight loss, in collaboration with primary care services.

Patients who are referred to this clinic attend three virtual consultations with endocrinologists over six to nine months, with access to support from dedicated diabetes nurse educators and dietitians while in the program.

We would be grateful if you would take some time to complete this survey, which seeks your views on a hospital-based diabetes remission service, and how this service can best meet your and your patients’ needs.
