Practical steps to Closing the Gap

This new Closing the Gap Improvement Workbook is a practical guide for practices, to enable them to better support Aboriginal and Torres Strait Islander people in their care.  

Closing the Gap aims to achieve health equality and to improve the health, wellbeing, life expectancy of Aboriginal and Torres Strait Islander people within a generation. There are more than 10,000 Aboriginal people in the north western Melbourne region. 

General practices play a pivotal role to diagnose, treat and help Aboriginal people to prevent and manage their health. For general practices making changes to support Closing the Gap include better patient identification, chronic disease prevention and management, and delivery of culturally appropriate care.  

The Closing the GAP QI workbook (an interactive PDF) will assist practices to undertake quality improvement activities using a simple step by step plan, templates and examples.   

It is a simple to use, self-guided tool for practices to track their progress and help provide best care.
