New end-of-life care quality improvement workbook for general practice

With the growth and ageing of Australia’s population, along with an increase of chronic and life-limiting illnesses, the types of patient groups requiring palliative care has widened. GPs are well-placed to provide end-of-life care for patients. 

North Western Melbourne Primary Health Network (NWMPHN) has released the End-of-Life Care Quality Improvement Workbook as the first in a series of Quality Improvement Workbooks aimed to help local practices on a range of topics.

The workbooks can be used as a self-guided quality improvement journey or with support from the NWMPHN.

This workbook provides a simple, four-step guide that draws on a proven approach, the Model for Improvement, with easy to follow activities and useful resources for end-of-life care.
