The Life! team is pleased to announce the continuation of the Life! program delivery through to 31 December 2020.
Life! is a lifestyle modification program funded by the State Government and coordinated by Diabetes Victoria. Delivered by qualified health professionals, the program teaches participants how to live a healthier life and reduce their risk of type 2 diabetes and cardiovascular disease.
Participants choose between a group course or telephone health coaching and learn how to improve their eating habits, physical activity, sleeping habits and stress management.
The Life! program provides a financial incentive for general practices to refer eligible patients to the program. The 2019-20 financial incentive agreement is now available.
