GPs: how do you use asthma action plans?

This information relates to the Asthma Action Plan Survey produced as part of a range of initiatives under the Victorian Department of Health funded Improving Childhood Asthma Management (ICAM) project.

The purpose of this survey is to help us understand how health care providers in primary care settings utilise written asthma action plans. Asthma Australia, the National Asthma Council Australia and the Royal Children’s Hospital Melbourne provide action plans that you may use or be familiar with. There are also a range of action plan templates produced by software and pharmaceutical companies.

As health care professionals you play a pivotal role in providing action plans and patient education on how to use them. We would like to hear your views on what improvements could be made to encourage uptake and adoption of asthma action plans.

Data collected via this survey is non-identifiable, but we do ask for a postcode. The results of the survey will be included in the ICAM project summary at completion of the program.
