North Western Melbourne Primary Health Network is looking for a GP to sit on a time-limited clinical advisory group to develop a new model of care for patients with complex chronic disease in collaboration with Royal Melbourne Hospital (RMH), cohealth and Merri Community Health.
The aim of the model is to improve care of these complex patients in the community where possible and reduce readmissions to RMH through easier access to services such as the Hospital Admission Risk Program, Hospital in the Home and specialist clinics for deteriorating patients who can’t be managed in the community.
