Darebin City Council’s Home and Community Care program for younger people assists Darebin residents under the age of 65 who live with a disability or are incapacitated in the short term. The program can also assist people who are not eligible for the NDIS or are awaiting/testing their eligibility for approval, to live as independently as possible in their homes and community.

There are different types of support depending on people’s needs. This might include assistance with daily activities, for example, shopping, personal care, dressing, meals, house cleaning, property maintenance, community access, and referrals to allied health services.

Once you have determined that your patient is an appropriate referral, we require some of the Service Coordination Tool Templates (SCTT pages) to be completed and sent back to us. If a person is self-referring, they do not have to complete the SCTT forms.
