Western Health Adult Diabetes Clinic would like to inform GPs of a change to our ‘Fail to attend’ policy. If a patient fails to attend their scheduled outpatient appointment without notice they will be discharged from our clinic.
Patients may seek to change the date of a scheduled appointment by contacting 8345 6490. If a discharged patient requires another appointment they will need to attend their general practitioner to obtain a new referral, and may be placed on the wait list.
General Practitioners can seek advice about the diabetes management of patients by contacting the Endocrine Registrar via switchboard on 8345 6666.
