Anticoagulant management post-discharge – brief survey from Western Health

Western Health is inviting GPs to participate in a study of anticoagulant management post-discharge. We are asking you to complete a brief (five-minute) survey.

As you are aware, anticoagulants are high risk medications which if prescribed and/or administered incorrectly can lead to serious patient harm. A major cause of hospital readmission is inadequate communication to health care providers on discharge. Western Health is committed to improving communication on discharge.

To help Western Health patient safety and communication to you, we would appreciate your time to answer a few short questions. Your responses are confidential and will be reported in an aggregated, de-identified format only.
