This webinar is part of a series focusing on Palliative Care, on the experience of specialist palliative care providers in recognising the important phases of a patient’s journey towards the end of life.

Guest presenters Dr Katie Tham and Dr Barbara Hayes will offer reflections on the importance of planning with patients for the end of life; how to have conversations; recording decisions; ensuring documentation is in place to support Goals of Care; how to address future care planning for people without capacity.
