Priority populationsNorth Western Melbourne Primary Health Network (NWMPHN) invites all residential aged care homes (RACHs) in its catchment to apply for grants of up to $20,000 (ex. GST) to upgrade virtual consultation facilities and technology.
The grants, funded by the Australian Government Department of Health and Aged Care, will enable residents to access clinically appropriate primary health care professionals through telehealth.
Grants can be used to:
- enhance wi-fi connection
- purchase telehealth equipment
- enhance telehealth facilities and/or consult rooms.
The initiative is a response to the final report of the Royal Commission into Aged Care Quality and Safety, published in March 2021.
How to apply
A limited amount of funding is available. Successful RACHs will receive grants of up to $20,000 (ex. GST).
RACHs will need to:
- meet the mandatory grant requirements
- complete a telehealth implementation plan (weighted score)
- complete a telehealth budget plan (weighted score).

Applications for round 2 funding will close at 5pm on Friday, 15 September 2023.
Applications will be open until the funding is exhausted and will be processed on a first come, first served basis, and assessed on a points-based system with a pass threshold. Early applications are thus advised.
