
Building multidisciplinary team care: expressions of interest sought from general practices in the Hume local government area
North Western Melbourne Primary Health Network (NWMPHN) is looking to fund three general practices in the Hume LGA to lead locally focused multidisciplinary team care for residents with chronic conditions.
The chosen practices will work closely with our Integration unit to build a care team structure tailored to the needs of their specific patient cohort. It will include allied health professionals and a dedicated care coordinator.
Each practice will be funded $66,600 (ex. GST) for participating in the project’s design, implementation and evaluation. There will be no cost to the practice or patient to access the care team.
Applications close at 12pm (AEST) Tuesday, 22 April 2025. Clarification questions must be submitted by 5pm, Wednesday, 16 April 2025.
Planning begins in July 2025. The model rolls out in August 2025 and concludes in mid-2028.
