Overview
The Risk Assessment, Recall and Referral (RARR) quality improvement project was a 12-month that aimed to prevent the onset of cardiovascular disease (CVD) and type 2 diabetes mellitus (T2DM). North Western Melbourne Primary Health Network commissioned 12 general practices to identify at-risk patients and provide them with lifestyle modification advice and referral to appropriate services.
Outcomes
The RARR project aimed to:
- increasing patient and practice awareness of CVD and T2DM risk
- increase understanding and uptake of patient risk assessments
- promote earlier detection, management, and referral to modifiable, lifestyle risk-reduction services and programs.
Project achievements included:
- 481 patients identified as at risk of developing CVD or T2DM
- 425 patients referred to lifestyle risk modification programs (LRMPs).
- 10 of the 12 practices increased their use of the heart health check MBS item (number 699) during the 6-month project activity phase. Combined results from all practices showed that use of this item increased from 91 in the six months prior to the project (November 2022 – April 2023), to 226 during the project (May 2023 – October 2023).
