Overview
Delivered by Youth Projects’ The Living Room, Parallel Pathways is a substance misuse treatment (SMT) service that provides holistic care to individuals experiencing alcohol and other drug challenges with mental health and homelessness comorbidities. The Living Room provides free health care and support to improve physical, mental, and social wellbeing of individuals who are homeless, or at risk of becoming so.
Services provided are counselling, case management, care planning and coordination, health promotion and psychosocial support.
Outcomes
- Integrated multidisciplinary care: The multidisciplinary team includes mental health, primary health and social work practitioners who work together with consumers on codesigned care plans to respond to comorbidities through all points of the recovery journey.
- A flexible holistic service: The program has established a ‘drop-in’ type service that takes a flexible, person-centred approach to case management, adapting to the needs of each individual.
- Tailored health promotion: Health promotion activities are designed and delivered based on the specific needs of the individual, ensuring that care is relevant and impactful.
- Social connection and skill building: The program offers strengths-based care, focusing on building social connections and life skills, particularly for individuals requiring short-term, low-intensity support to connect with their local communities.
- Psychosocial funding grant: The program has received two rounds of psychosocial grants for 2023-24 and 2024-25. This is designed to enhance the capacity of the AOD program, specifically to support individuals with psychosocial issues and AOD comorbidities.
