Overview
Jesuit Social Services Connexions is a voluntary program that delivers AOD outreach, support and advocacy to young people aged 16-28 with AOD and mental health co-occurring needs and who have disengaged from mainstream services and have experienced homelessness, family violence, un or underemployment and interacted with child protection, youth or adult justice systems. Family support is also included.
Services include case management, care planning and coordination, assertive outreach, advocacy, post rehabilitation support and relapse prevention, counselling and psychosocial services.

Outcomes
- Increased resilience and trust: Young people are empowered with essential life skills and develop trusting relationships to build resilience, enhancing their ability to navigate daily life challenges.
- Tailored therapeutic support: Participants receive multifaceted support that addresses both immediate AOD concerns and broader social determinants of health through collaboration with complementary services fostering a more stable environment for young people in their recovery journey.
- Advocacy and collaborative partnerships: Integrates advocacy highlighting service gaps to build positive partnerships with aligning services, helping to ensure participants have better access to inclusive and responsive services.
- Psychosocial funding grant: The program has received two rounds of psychosocial grants for 2023-24 and 2024-25. This is designed to enhance the capacity of the AOD program, specifically to support individuals with psychosocial issues and AOD comorbidities.
