Overview
The Check-In program is a youth enhanced service, based at Melbourne City Mission’s Frontyard Youth Services. Run in partnership with St Vincent’s Hospital, it provides holistic care and support to people aged 12-25 who present with complex mental health and social health needs and are experiencing, or at risk of experiencing, homelessness.
Providing clinical and non-clinical support and interventions, the multidisciplinary team comprises of specialist mental health peer workers, occupational therapists, care coordinators and dual diagnosis workers. The team provides centre-based support and community-based outreach, providing improved connection to shorter- and longer-term mental health goals and recovery plans with young people.
The program offers trauma-informed support, addressing client needs through consistent care, integrated services and practical supports to enhance mental health and wellbeing.
The program is strengthened through its partnership with St Vincent’s Hospital Melbourne.
The service is provided free of charge.
Outcomes
The Check-In program is designed to:
- Provide early access for young people with or at risk of severe mental illness.
- Recognise and address co-occurring needs such as problematic alcohol and other drug issues and homelessness. It facilitates care navigation and coordination with additional services to deliver integrated care. Achievements are measured using the customer relationship management system “Passport” and case studies, demonstrating successful management of these complex needs and improved service transitions.
- Support flexible and responsive service access, particularly for underserved young people. The program is successful in providing adaptable service access, with positive feedback from young people highlighting the flexibility and responsiveness of support.
- Improve care integration through partnerships, notably with St Vincent’s Hospital. This partnership aims to ensure young people receive the right care at the right time and place. Achievements show enhanced collaboration, reduced service incidents, and better alignment of care services.
