Use the QI template below to boost immunisations for under- or unvaccinated children less than 5-years-old who have been seen by your practice.
Goal
Decrease the number of children 5 and under not up-to-date with childhood immunisations.
Measures
Number of children 5 and under recorded as not up-to-date in AIR reports will decrease.
Improvement ideas
- Identify children 5 and under who are not up-to-date with childhood immunisations.
- Use the Australian Immunisation Register (AIR) to identify children who have attended the practice and are overdue for immunisation.
- Decide as a team how we will contact these patients – for instance, through SMS recall, direct phone call, email, or opportunistically at appointments.
- Develop appointment triggers to raise immunisation with patients and their families.
