Number of ACP discussions recorded in clinical software.

Improvement ideas

Decide on a method for recording ACP discussions in clinical software.
Use PenCAT4 to identify all active patients with at least one chronic condition who are eligible for a medication review or patients eligible for an annual 75+ health assessment.
Offer ACP resources in a patient’s primary language.
